Here's the simple version: both nebivolol and Rauvolfia vomitoria push your blood pressure in the same direction, which is down. Nebivolol is a beta blocker that already slows your heart rate and relaxes your blood vessels. Rauvolfia contains a small amount of reserpine, which lowers blood pressure by depleting nerve signals that keep your vessels tightened. Stack the two together, and the effects can add up.

If they pile on, you might feel more lightheaded, dizzy, tired, or faint, especially when standing up quickly, and your pulse could run slow. To be fair, this is a theoretical concern based on how these compounds work, not strong proof from studies. Still, please check with your pharmacist or doctor before combining them so they can keep an eye on your blood pressure.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the gist: your body relies on a liver enzyme called CYP2D6 to break down nebivolol. Rauvolfia vomitoria contains a small amount of yohimbine, and lab studies suggest yohimbine can slow that enzyme down. If that happens, nebivolol could clear more slowly and build up, leading to stronger blood-pressure and heart-rate lowering than you expect.

In real life that might show up as dizziness, lightheadedness, unusual tiredness, or a slower-than-normal heartbeat. I'll be honest though, this concern comes only from test-tube research, not from studies in people, so it's theoretical. On top of that, Rauvolfia itself lowers blood pressure, so the two together could stack up. Please check with your pharmacist or doctor before pairing them.
