MONOAMINE OXIDASE INHIBITORS (MAOIs)
Why this applies to Safinamide Mesylate
Safinamide Mesylate falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Rauvolfia Vomitoria interacts with.
Yohimbine, a constituent of Rauvolfia vomitoria, has MAO inhibitory effects. At high doses, yohimbine is a non-selective inhibitor of MAO.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the concern with pairing Rauvolfia vomitoria and Xadago (safinamide). Safinamide is already an MAO-B inhibitor, and rauvolfia contains compounds (like yohimbine) that can also block MAO enzymes, especially at higher doses. So you'd essentially be stacking two things that work on the same pathway. On top of that, rauvolfia strongly affects blood pressure and the brain's chemical messengers, which makes the combination riskier.
In real life, that overlap could push your blood pressure up or down unpredictably, and in theory raise the chance of a dangerous spike or serotonin-related symptoms (agitation, sweating, racing heart, confusion). This is rated as a theoretical interaction based on pharmacology, not confirmed cases, but because it's potentially serious I'd treat it cautiously. Please don't combine these without checking with your doctor or pharmacist first.

Safinamide (brand name Xadago) is used as an add-on treatment to levodopa/carbidopa in people with Parkinson’s disease who have “off” episodes. “Off” episodes are times when Parkinson’s symptoms return or get worse between doses of your usual medicine.
Xadago has been shown to work only when taken together with levodopa/carbidopa.
