Tramadol, Acetaminophen falls under CNS DEPRESSANTS — the group Theanine interacts with.

Theoretically, theanine might have additive sedative effects when used in conjunction with CNS depressants. However, it is unclear if this concern is clinically relevant.
Theoretically, theanine may compete with glutamate and/or increase plasma gamma-aminobutyric acid (GABA) levels, which could cause CNS depression. In one clinical study, some subjects taking oral theanine reported drowsiness.

Here's the gist with Tramacet (which combines tramadol and acetaminophen) and theanine. The tramadol part can make some people feel drowsy, foggy, or relaxed on its own. Theanine is thought to gently nudge calming brain chemicals like GABA, and a few people in studies noticed mild sleepiness from it. So in theory, taking the two together could add up to a little extra drowsiness.

That said, this is a theoretical concern based on how each works, not something well documented in real patients. It's rated minor and unlikely. Theanine doesn't change how your body breaks down tramadol, so it won't make the painkilling effect stronger or weaker. If you do try both, just notice how alert you feel before driving, and check with your pharmacist or doctor first.

Clinical studies regarding the effects of L-theanine on serotonin levels are conflicting. Some studies suggest it can increase serotonin levels in the brain while others report that it may decrease them. Nevertheless, there have been no reports of l-theanine being a causative agent in serotonergic-related side effects or serotonin syndrome.

Here's the short version: this pairing is very low on my worry list. Tramacet contains tramadol, which gently nudges serotonin upward in the brain, and theanine might tweak serotonin too. On paper, stacking two things that touch serotonin raises a flag for serotonin syndrome (think agitation, racing heart, sweating, shivering, or muscle twitching). But the research on theanine and serotonin is genuinely mixed, with some studies showing a rise and others a drop.

In real life, there are no reports of theanine causing serotonin problems, and the evidence here is weak. The acetaminophen side adds nothing to this concern. I wouldn't lose sleep over it, but since tramadol is already serotonin-active, give your pharmacist a quick heads-up before combining them.
