Theoretically, the caffeine in yerba mate might reduce the effects of phenobarbital and increase the risk for convulsions.
Yerba mate contains caffeine. Animal research suggests that caffeine can decrease the anticonvulsant activity of phenobarbital. However, the exact mechanism of this interaction is unclear.

Here's the gist: phenobarbital is often used to calm the nervous system and prevent seizures. Yerba mate, on the other hand, is loaded with caffeine, which does the opposite, it stimulates the brain. So in theory, the caffeine could push against what your phenobarbital is trying to do and make it a little less effective at keeping seizures in check.

I want to be honest about how solid this is. The concern comes from animal studies, not strong human data, and the exact way it happens isn't well understood. A casual cup now and then likely isn't a big deal, but regular, heavy mate drinking is worth mentioning to your doctor or pharmacist, especially if your seizures aren't fully controlled.
