Cevimeline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the basic idea with these two. Your body relies on a liver enzyme called CYP2D6 to help clear cevimeline (Evoxac) out. Yohimbe contains yohimbine, which in lab studies appears to slow that same enzyme down. If cevimeline isn't cleared as quickly, more of it can hang around, and that could nudge its usual effects up a notch. With cevimeline, that might mean more sweating, nausea, runny nose, watery eyes, or needing the bathroom more often.

I'd keep this in perspective, though. The evidence here comes only from test-tube work, not real patients, so it's theoretical. Still, since over-the-counter yohimbe can raise blood pressure and heart rate on its own, it's worth a quick chat with your pharmacist or doctor before combining the two.

Here's the gist for these two together. Your body uses a liver enzyme called CYP3A4 to help break down cevimeline (Evoxac). Lab studies suggest yohimbe may speed that enzyme up a bit, which in theory could clear cevimeline a little faster and leave less of it working. Since cevimeline is the active drug itself (not a prodrug), faster clearance could mean it works a touch less well, so your dry-mouth relief might not be as strong.

I'll be honest, this is a theoretical concern based only on test-tube research, not real patient cases, so I wouldn't lose sleep over it. Yohimbe has its own risks though, like raising blood pressure and heart rate. Please run it by your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
