Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Yohimbe interacts with.

Theoretically, yohimbe might reduce the effects of antihypertensive drugs.
Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control.

Here's the simple version. Iloprost works by relaxing and widening your blood vessels, including those in the lungs, to ease the pressure of pulmonary arterial hypertension. Yohimbe contains yohimbine, which tends to tighten blood vessels and can push blood pressure and heart rate up. So the two work in opposite directions, and yohimbe could blunt some of iloprost's benefit.

I want to be honest that this concern is theoretical, based on how each one behaves rather than on direct studies. Still, over-the-counter yohimbe products are unpredictable and known to cause spikes in blood pressure and a racing heart, which is the last thing you want when you're managing PAH. Please don't start yohimbe without first checking with your doctor or pharmacist.

Iloprost also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, combining yohimbe bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

Here's the thing to know about pairing yohimbe with iloprost (Ventavis): both can nudge your platelets toward sticking together less. Iloprost already has some mild blood-thinning, anti-clotting activity as part of how it relaxes vessels, and lab studies suggest yohimbine (the active piece of yohimbe) can also slow platelets from clumping. Put together, the theoretical worry is a little extra bleeding tendency, like easier bruising or longer bleeding from a small cut.

I want to be honest that this is weak, test-tube level evidence and hasn't actually shown up as a problem in people. The bigger real-world concern with yohimbe is its tendency to raise blood pressure and heart rate, which works against what iloprost is trying to do for your lungs. Please run it by your pharmacist or doctor first.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

Brand names: Ventavis, Aurlumyn

Aurlumyn (iloprost injection, given intravenously) is approved to treat severe frostbite in adults, specifically to reduce the risk of losing fingers or toes (digit amputations). Its effectiveness was established in young, healthy adults who experienced frostbite at high altitudes.
