Here's the gist for Darvon N (propoxyphene), an older opioid pain reliever. Your liver uses an enzyme called CYP2D6 to help break it down. Lab studies hint that yohimbine, the active piece of yohimbe, can slow that same enzyme. If it does, propoxyphene could clear out more slowly and build up a bit, which might mean stronger opioid effects like drowsiness, dizziness, nausea, constipation, or slowed breathing.

I want to be honest though: this is only a theoretical concern from test-tube research, not from real patients, so the actual risk is uncertain. Yohimbe on its own can also raise blood pressure and heart rate. Before pairing these two, please run it by your pharmacist or doctor first.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the key point with this pair: it's not your propoxyphene that builds up, it's the yohimbine. Propoxyphene blocks the liver enzyme CYP2D6, and yohimbine happens to be cleared by that same enzyme. So when the two are taken together, yohimbine can hang around longer and reach higher levels than it would on its own.

Why that matters: yohimbine on its own can already spike your blood pressure, race your heart, and trigger anxiety, sweating, or the jitters. Slowing its breakdown can make those effects stronger. On top of that, propoxyphene is an older opioid pain reliever with its own heart and sedation risks. I'd really encourage you to run this combo past your pharmacist or doctor before using yohimbe.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
