Here's the wrinkle with hydrocodone: your liver uses an enzyme called CYP2D6 to turn it into a more potent painkiller called hydromorphone. That conversion step is part of how it eases pain. Lab studies suggest yohimbine, the active piece of yohimbe, can block CYP2D6. In theory, that could mean a bit less of the strong active form is made, so your pain relief might feel weaker than usual.

I want to be honest about how soft this evidence is. It comes only from test-tube work, not from real people, so we can't say how much it actually matters day to day. Yohimbe also carries its own risks, like raising blood pressure and heart rate. Before pairing these, please check with your pharmacist or doctor first.

Here's the gist for these two together. Yohimbe may slightly speed up one of your liver's clean-up enzymes, called CYP3A4, and your body uses that same enzyme to help process hydrocodone. If the enzyme works a bit faster, hydrocodone could be cleared a little quicker, which in theory might leave you with less pain relief than usual.

I want to be straight with you though: this is a weak, theoretical concern based only on lab studies, not real people, and it's rated minor. The bigger issue with yohimbe is its own well-known habit of raising blood pressure and speeding up your heart rate. Since you're on hydrocodone for serious pain, please run yohimbe past your pharmacist or doctor before starting it.
