Thioridazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the basic idea with these two together. Your body relies on a liver enzyme called CYP2D6 to clear thioridazine out of your system. Lab studies hint that yohimbine, the active part of yohimbe, can slow that enzyme down. If thioridazine isn't broken down as quickly, more of it can linger in your blood, which may amplify its effects and side effects.

That matters here because thioridazine already carries real heart risks. Higher levels can mean more drowsiness, dizziness, and a dangerous change in heart rhythm called QT prolongation. On top of that, yohimbe itself can spike blood pressure and heart rate. To be fair, this is only theoretical from test-tube work, not proven in people, but given thioridazine's heart concerns I'd want you to clear it with your pharmacist or doctor first.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the thing to know about combining yohimbe with thioridazine (Mellaril). Thioridazine blocks the liver enzyme CYP2D6, which is the very same enzyme your body uses to clear yohimbine, the active part of yohimbe. With that cleanup slowed down, yohimbine can build up to higher levels and stick around longer than it normally would.

That matters because yohimbe on its own can already push up your blood pressure and heart rate and cause anxiety, jitteriness, and sweating. Higher levels can make those effects stronger. On top of that, thioridazine itself can affect your heart rhythm, so adding a stimulant-like supplement is a combination I'd want you to clear with your pharmacist or doctor before trying anything.

Yohimbine, a constituent of yohimbe, has alpha-2 adrenergic antagonist effects. Theoretically, combining it with phenothiazines can cause additive alpha-2 adrenergic antagonism.

Here's the gist for combining yohimbe with thioridazine (Mellaril). Both can tug on the same nerve receptors (alpha-2 adrenergic receptors). Thioridazine already blocks some of these as part of how it works, and yohimbine, the active piece of yohimbe, blocks them too. Stacking them together could add up and theoretically affect things like blood pressure and heart rhythm. With thioridazine that's worth extra caution, since this drug is already known for affecting heart rhythm (QT prolongation) on its own.

To be honest, this concern is theoretical, based on how the two work rather than solid studies in people. Still, since over-the-counter yohimbe is unpredictable and thioridazine is a serious medication, please check with your pharmacist or doctor before mixing them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. What Is Thioridazine Used For?

Brand name: Mellaril

Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.

Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
