Perphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the gist with these two. Your body relies on a liver enzyme called CYP2D6 to help clear perphenazine out of your system. Lab studies suggest yohimbine (the active piece of yohimbe) can slow that enzyme down. If it does, perphenazine could build up a bit higher than usual, which may mean stronger effects and more side effects like drowsiness, dizziness, low blood pressure, or muscle stiffness and restlessness.

I want to be honest though, this is based only on test-tube research, not real people, so it's more of a theoretical concern. There's also a separate worry: yohimbe itself can raise blood pressure and heart rate. Before combining them, please check with your pharmacist or doctor first.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the key thing to know: your body relies on a liver enzyme called CYP2D6 to clear yohimbine, the active compound in yohimbe. Perphenazine can block that same enzyme, which means yohimbine may not get broken down as quickly. The result is that yohimbine can build up to higher levels than expected, even from a normal dose.

Theoretically, using yohimbine with phenothiazines might have additive effects.
Yohimbine, a constituent of yohimbe, has alpha-2 adrenergic antagonist effects. Theoretically, combining it with phenothiazines can cause additive alpha-2 adrenergic antagonism.

Here's the situation if you're thinking about pairing yohimbe with your perphenazine (Trilafon). Both of these touch the same kind of receptor in your body, called the alpha-2 adrenergic receptor. Yohimbe's active compound, yohimbine, blocks it, and perphenazine has some blocking action on similar receptors too. Stack them together and that effect could pile up.

What might that feel like? Things like dizziness, lightheadedness when you stand up, a racing heart, or jumpiness and anxiety. Keep in mind this is a theoretical concern based on how the drugs work, not something proven in studies, so I don't want to alarm you. Still, yohimbe can be unpredictable on its own. Please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

What Is Perphenazine Used For?

Brand name: Trilafon

Perphenazine tablets are approved to treat schizophrenia in adults. They are also used to control severe nausea and vomiting in adults when other measures are not enough.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
