Here's the gist for propafenone (Rythmol). Your liver uses an enzyme called CYP2D6 to clear this medication. Lab studies suggest yohimbine, the active part of yohimbe, can slow that same enzyme down. If that happens in real life, propafenone could build up in your body and hit harder than intended.

With a heart rhythm drug, that matters. Higher levels could mean more dizziness, a slow or pounding heartbeat, or new rhythm problems. On top of that, yohimbe itself can raise blood pressure and speed up your heart, which works against what propafenone is trying to do. The evidence here is only from test-tube research, so this is theoretical, not proven. Still, with a heart medication I'd play it safe and check with your pharmacist or doctor before combining them.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the key thing about propafenone: it blocks an enzyme in your body called CYP2D6, and that same enzyme is the one responsible for clearing yohimbine (the active part of yohimbe) out of your system. So when you take them together, the yohimbine isn't broken down as quickly and can build up to higher levels than expected.

What does that mean for you? Mostly it ramps up yohimbine's own side effects, things like a racing or pounding heart, spikes in blood pressure, anxiety, sweating, and jitteriness. Since you're already on a medication to steady your heart rhythm, adding a stimulant-like compound that can stir things up is worth a careful conversation. Here's the gist for propafenone (Rythmol). Your liver uses a few enzymes to clear this drug, and one of them is called CYP1A2. In lab testing only, yohimbe appeared to speed that enzyme up. If that happened in real life, your body might clear propafenone a little faster, which could mean slightly lower levels and a weaker hold on your heart rhythm. In theory, that could let an irregular heartbeat or palpitations sneak back.

I want to be honest though: this is rated minor and rests only on test-tube data, so it may not matter much in everyday use. Still, because propafenone is a heart-rhythm drug and yohimbe itself can raise blood pressure and heart rate, please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Propafenone

What Is Propafenone Used For?

Brand names: Rythmol, Arythmol

Propafenone is used to help control abnormal heart rhythms. It is prescribed to delay the return of paroxysmal atrial fibrillation or atrial flutter (episodic irregular heartbeats in the upper chambers) and paroxysmal supraventricular tachycardia (PSVT, sudden fast heartbeats starting above the heart's lower chambers) — but only in people who do not have structural heart disease.

It is also used to treat life-threatening ventricular arrhythmias (dangerous fast rhythms starting in the heart's lower chambers). Because of serious safety concerns, it is not recommended for minor or non-life-threatening heart rhythm problems.
