Ranolazine falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS — the group Yohimbe interacts with.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the key thing to understand: this interaction isn't really about ranolazine building up. It works the other way around. Ranolazine is a known blocker of an enzyme called CYP2D6, which is the same enzyme your body uses to clear yohimbine (the active part of yohimbe). So when you take them together, the yohimbine can hang around longer and reach higher levels than expected.

That matters because yohimbe on its own can already push up your blood pressure and speed up your heart rate. Adding ranolazine may amplify those effects, which is a real concern when you're already managing heart-related chest pain. I'd hold off and check with your pharmacist or doctor before combining them.

Here's the gist for ranolazine (Ranexa). Your body relies heavily on an enzyme called CYP3A4 to clear this medicine, and lab research suggests yohimbe may speed that enzyme up. If that happened in real life, your system could break down ranolazine a bit faster, meaning lower levels and possibly less relief from your angina (the chest pressure could creep back).

I want to be straight with you though: this is only a theoretical concern from test-tube studies, not something proven in people, and it's rated as minor. Still, the bigger issue is yohimbe itself, since it can spike your blood pressure and heart rate, which is rough on anyone managing heart-related chest pain. Please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
