Fentanyl citrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the situation with combining yohimbe and fentanyl. Lab studies hint that yohimbine, the active compound in yohimbe, may slow down a liver enzyme called CYP2D6. In theory, if that enzyme is blocked, a drug it helps clear could build up in your body. With a powerful opioid like fentanyl, more drug hanging around could mean stronger effects such as heavy sedation, slowed breathing, confusion, or dangerously low blood pressure.

That said, I want to be honest about the evidence: this is theoretical and comes only from test-tube research, not real patients. Fentanyl also relies more on a different enzyme (CYP3A4) than CYP2D6. Still, fentanyl leaves little room for error, so please check with your doctor or pharmacist before mixing these two.

Here's the gist for fentanyl: your liver uses an enzyme called CYP3A4 to break it down. Lab studies hint that yohimbe might speed up that enzyme, which in theory could clear fentanyl from your body a little faster and make it less effective. For a strong pain medicine like fentanyl, weaker pain control is the main thing you'd notice. Fentanyl is active on its own (not a prodrug), so this direction makes sense.

The good news is this concern comes only from test-tube research, not from real patients, so it's rated minor and theoretical. Still, yohimbe carries its own real risks like racing heart and high blood pressure. Please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.
