Ifosfamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.

Here's the wrinkle with ifosfamide: it's actually a prodrug. Your liver uses an enzyme called CYP3A4 to switch it on into the form that attacks cancer cells. Lab studies suggest yohimbe may speed up (induce) that same enzyme.

So in theory, yohimbe could shift how ifosfamide is processed. The bigger concern isn't a weaker drug, it's that faster CYP3A4 activity can produce more of ifosfamide's toxic byproducts, which are linked to side effects like bladder irritation and confusion or drowsiness. That said, this is only based on test-tube research, not real patients, so we don't truly know what happens in people. Since ifosfamide is serious chemo, please don't add yohimbe without first clearing it with your oncology team.
