Ketoconazole falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Yohimbe interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the gist of it: ketoconazole is a strong blocker of a liver enzyme called CYP3A4, and that same enzyme is what your body uses to clear yohimbine, the active compound in yohimbe bark. When you slow that enzyme down with ketoconazole, the yohimbine can hang around longer and build up higher than expected.

That matters because yohimbe's main side effects are things like a racing heartbeat, spikes in blood pressure, anxiety, jitteriness, and trouble sleeping, and higher levels could make those worse. To be fair, this concern is theoretical, based on how the two are processed rather than on proven cases. Still, since over-the-counter yohimbe doses vary so much, please check with your pharmacist or doctor before combining them.

Here's the idea behind this pairing. Your body clears oral ketoconazole partly through a liver enzyme called CYP3A4. Some lab studies suggest yohimbe may speed up that enzyme, which could move ketoconazole out of your system a little faster. In theory, that might lower ketoconazole levels and make it work less well against your fungal infection.

I want to be honest about how thin this evidence is. It comes only from test-tube research, it's rated a minor interaction, and it has not been shown in real people. So I wouldn't lose sleep over it, but yohimbe on its own can raise blood pressure and heart rate, which is reason enough to check with your pharmacist or doctor before combining the two.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

What Is Ketoconazole Used For?

Brand name: Nizoral

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.

This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
