Here's the key thing to understand: in this pairing, it's the yohimbe that may get affected, not so much your larotrectinib. Yohimbine (the active part of yohimbe) is broken down by a liver enzyme called CYP3A4, and larotrectinib can mildly block that same enzyme. So if you take them together, yohimbine could build up in your body and hit harder than expected.

What that looks like in real life is yohimbe's known side effects getting worse, things like a racing heartbeat, spikes in blood pressure, anxiety, or jitteriness. I'll be honest, this is a theoretical concern based on how the drugs work, not something proven in studies. Still, since yohimbe is risky on its own and you're already managing cancer treatment, please run this by your oncology team or pharmacist before combining them.

Here's the short version for Vitrakvi (larotrectinib): your body clears this cancer drug using a liver enzyme called CYP3A4. Lab studies hint that yohimbe may rev up (induce) that enzyme. If that happened in real life, your body could clear larotrectinib a little faster, which might lower its blood levels and, in theory, make it work less well against your tumor.

I want to be honest about how soft this evidence is. It comes only from test-tube research, not from people, and it's rated as a minor, possible interaction. Still, with a cancer medicine you don't want anything chipping away at how well it works. Yohimbe also carries its own risks like high blood pressure and a racing heart. Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Larotrectinib

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration that can drive tumor growth. It is used when the tumor is metastatic (has spread) or when surgery to remove it would likely cause serious harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients must be selected for Vitrakvi therapy based on an FDA-approved test that confirms the NTRK gene fusion in their tumor. This approval is under the FDA's accelerated approval pathway, based on how many patients responded to treatment and how long those responses lasted. Continued approval may depend on results from follow-up clinical trials.
