Netupitant, Palonosetron (Oral) falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Yohimbe interacts with.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know with this pairing: one of the ingredients in Akynzeo, netupitant, gently blocks an enzyme called CYP3A4. That same enzyme is what your body uses to break down yohimbine, the active compound in yohimbe. So while you're taking Akynzeo, your body may clear yohimbine more slowly, letting it build up a bit higher than usual.
If that happens, you could feel yohimbe's known side effects more strongly, things like a racing heartbeat, jitteriness, anxiety, and elevated blood pressure. I'll be honest, this is a theoretical concern based on how these compounds are processed, not something proven in studies. Still, yohimbe can be unpredictable on its own, so please check with your pharmacist or doctor before combining the two.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Akynzeo (netupitant/palonosetron) and yohimbe. The netupitant part is cleared by a liver enzyme called CYP3A4. Lab studies (test tube only, not people) suggest yohimbe might speed that enzyme up. If that happened in the body, netupitant could be broken down a little faster, which could in theory make it slightly less effective at keeping nausea and vomiting away during your chemo cycle.
I want to keep this in perspective. This is a theoretical, minor concern based only on lab data, so it's far from certain. Still, the bigger issue is that yohimbe itself can raise blood pressure and speed up your heart, which isn't great while you're going through chemo. Please run it by your oncology team or pharmacist before using it.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. What Is Netupitant and Palonosetron Used For?
Brand name: Akynzeo
Netupitant and palonosetron helps prevent nausea and vomiting from cancer chemotherapy in adults. It is used together with dexamethasone, a steroid medicine. It covers both the first day (acute phase) and the days after (delayed phase).
Akynzeo capsules are used for initial and repeat courses of chemotherapy, including highly emetogenic (strongly nausea-causing) chemotherapy. Akynzeo for injection and Akynzeo injection are used for highly emetogenic chemotherapy only. They have not been studied with anthracycline plus cyclophosphamide chemotherapy.
