Yohimbine, a constituent of yohimbe, has alpha-2 adrenergic antagonist effects. Theoretically, combining it with phenothiazines can cause additive alpha-2 adrenergic antagonism.

Here's the gist for Promazine (Sparine), an older phenothiazine antipsychotic. Like its drug family, promazine can block alpha-1 adrenergic receptors, which tends to relax blood vessels and lower blood pressure. The yohimbine in yohimbe works on a related receptor (alpha-2) and can stir up the nervous system, sometimes raising blood pressure and heart rate. Stacking the two could pull your body in opposite, unpredictable directions.

Honestly, this is a theoretical concern based on how these compounds behave, not on solid studies, so I wouldn't panic. Still, you might notice dizziness, lightheadedness when standing, or a racing heart. Before mixing them, please run it by your pharmacist or doctor first.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
