Promethazine falls under PHENOTHIAZINES — the group Yohimbe interacts with.

Theoretically, using yohimbine with phenothiazines might have additive effects.
Yohimbine, a constituent of yohimbe, has alpha-2 adrenergic antagonist effects. Theoretically, combining it with phenothiazines can cause additive alpha-2 adrenergic antagonism.

Here's the gist for these two together. Promethazine (Phenergan) is an antihistamine that also blocks some alpha-adrenergic receptors, and yohimbe's active piece, yohimbine, blocks alpha-2 receptors too. Stacking them could nudge your blood pressure and heart rate around in unpredictable ways. You might feel more lightheaded or dizzy, especially when standing up, or notice a faster, pounding heartbeat from the yohimbe side.

I want to be straight with you: this is a theoretical concern based on how each works, not something proven in studies, so don't panic. Yohimbe on its own can already spike blood pressure and heart rate, which is reason enough to be cautious. Before combining them, please run it by your pharmacist or doctor first.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Promethazine

What Is Promethazine Used For?

Brand name: Phenergan

Promethazine is used for a wide range of conditions. Tablets, oral solution, and rectal suppositories (brands including Phenergan, Promethegan, and Phenadoz) are approved for seasonal and perennial allergic rhinitis, vasomotor rhinitis, allergic conjunctivitis, hives (urticaria), angioedema, dermographism, and allergic reactions to blood or plasma. They are also used for motion sickness (both prevention and active treatment), nausea and vomiting related to anesthesia and surgery, sedation, and as a helper medication alongside pain relievers after surgery.

Promethazine hydrochloride injection (brands including Phenergan and Promethazine HCl injection) is used for many of the same purposes but is reserved for situations where oral treatment is not possible — including allergic emergencies as an adjunct to epinephrine, sedation during labor, and as an aid during certain surgical procedures such as bronchoscopy and eye surgery.
