Treprostinil falls under ANTIHYPERTENSIVE DRUGS — the group Yohimbe interacts with.

Theoretically, yohimbe might reduce the effects of antihypertensive drugs.
Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control.

Here's the thing to know about Orenitram (treprostinil): it works partly by relaxing and widening your blood vessels, which helps lower the pressure in the arteries of your lungs. Yohimbe pushes in the opposite direction. The yohimbine in it can raise blood pressure and speed up your heart, so taking it alongside treprostinil could work against the very effect you're relying on this medication for.

To be fair, this concern is theoretical, based on how yohimbine behaves in the body rather than studies pairing these two directly. Still, since treprostinil is a serious medication for a serious condition, I wouldn't gamble with it. Please check with your pharmacist or doctor before adding yohimbe, and don't change your treprostinil on your own.

Theoretically, combining yohimbe bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

Treprostinil (Orenitram) is a prostacyclin medicine used for pulmonary arterial hypertension. Along with widening blood vessels in the lungs, it naturally makes platelets a little less sticky, so it does carry some bleeding risk on its own. The concern here is that yohimbine, the active piece of yohimbe, has shown a similar platelet-slowing effect in lab studies. Stacking the two might add up and tip you toward easier bruising or bleeding.

Honestly, this is theoretical and only seen in test-tube work, so I wouldn't lose sleep over it. The bigger issue is that yohimbe can spike your blood pressure and heart rate, which isn't ideal with a heart and lung condition. Please run it by your doctor or pharmacist before trying it.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
