Anagrelide falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Guarana interacts with.

Theoretically, guarana may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that guarana extract can inhibit platelet aggregation. This effect may be due to the caffeine in guarana, which is also reported to have antiplatelet activity. This interaction has not been reported in humans.

Here's the simple version. Anagrelide already lowers your platelet count and makes them a little less sticky, so your blood is more prone to bleeding to begin with. Lab and animal studies hint that guarana (mostly because of its caffeine) can also keep platelets from clumping together. Stack the two and, in theory, you could nudge your bleeding risk up a bit.

The honest caveat: this has only been seen in test tubes and animals, never confirmed in real people, so it's more of a 'keep an eye on it' than an alarm. Still, watch for things like easy bruising, nosebleeds, bleeding gums, or cuts that take longer to stop. Guarana also adds caffeine on top of anagrelide, which can already cause a fast or pounding heartbeat. Please check with your pharmacist or doctor before adding it.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the key thing to understand: this interaction is really about the caffeine inside guarana, not about your anagrelide getting stronger or weaker. Anagrelide can mildly slow down the CYP1A2 enzyme, which is the same enzyme your body uses to clear caffeine. So in theory, the caffeine from guarana may hang around longer and build up a bit more than usual.

What that can feel like is the usual too-much-caffeine stuff: a racing or pounding heart, jitteriness, trouble sleeping, or feeling anxious. That matters a little extra here because anagrelide itself can already speed up your heart and cause palpitations, so stacking caffeine on top could make those symptoms more noticeable. That said, this is a minor, theoretical concern based mostly on lab data, not strong human studies. Just check with your pharmacist or doctor before adding concentrated guarana supplements.

What Is Anagrelide Used For?

Brand name: Agrylin

Anagrelide — sold as Agrylin and as generic anagrelide capsules — is used to treat thrombocythemia secondary to myeloproliferative neoplasms. That means it's prescribed when a bone marrow disorder causes the body to produce too many platelets, raising the risk of dangerous blood clots.

By bringing platelet counts down to a safer range, anagrelide also helps reduce related symptoms, including bleeding and clotting events (thrombo-hemorrhagic events).
