Antithrombin III falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Guarana interacts with.

Theoretically, guarana may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that guarana extract can inhibit platelet aggregation. This effect may be due to the caffeine in guarana, which is also reported to have antiplatelet activity. This interaction has not been reported in humans.

Antithrombin III (Thrombate III) is a blood-thinning protein given by IV to help prevent dangerous clots, often around surgery or childbirth in people with a genetic antithrombin shortage. Guarana is basically a concentrated source of caffeine, and lab and animal studies hint that this caffeine may make platelets a little less sticky. In theory, stacking that mild effect on top of a blood thinner could nudge your bleeding risk up.

I want to be honest with you here: this interaction has only been seen in test tubes and animals, never reported in actual patients, so it's mostly theoretical. Still, because you're on a clotting medication, it's worth flagging things like easy bruising or unusual bleeding to your care team. Please check with your doctor or pharmacist before adding a guarana supplement.
