Why this applies to Cablivi
Cablivi falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Guarana interacts with.
In vitro and animal research suggests that guarana extract can inhibit platelet aggregation. This effect may be due to the caffeine in guarana, which is also reported to have antiplatelet activity. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Cablivi (caplacizumab) is a strong medicine that blocks platelets from sticking together, which is why your care team uses it for aTTP. The concern here is simple: guarana is basically concentrated caffeine, and lab and animal studies hint that it too may make platelets a little less sticky. Stack a mild blood-thinning effect on top of an already powerful one, and on paper you could see a higher chance of bleeding.
I want to be honest about how shaky this evidence is. This effect has only shown up in test tubes and animals, never in real patients, so it's theoretical. Still, with Cablivi watch for easy bruising, nosebleeds, bleeding gums, or unusual bleeding. Please loop in your doctor or pharmacist before adding guarana or any caffeine pills while you're on Cablivi.

Caplacizumab-yhdp injection is used to treat acquired thrombotic thrombocytopenic purpura (aTTP; a disorder in which the body attacks itself and causes clots, low amounts of platelets and red blood cells, and could cause other serious complications) in combination with plasma exchange therapy and immunosuppressant medications. Caplacizumab-yhdp is in a class of medications called antithrombotic agents. It works by blocking the action of a certain substance in the body that causes the symptoms of aTTP.
