Glucovance is a two-in-one diabetes pill: glyburide nudges your pancreas to release more insulin, and metformin helps your body use insulin better. Neither one is a prodrug that depends on guarana to work, so this isn't about guarana switching the medicine on or off. The real question is what the caffeine in guarana does to your blood sugar.

Honestly, the evidence here is shaky and mixed. Some reports suggest caffeine can bump blood sugar up a little, others suggest the opposite, so it's hard to predict. For most people the effect is small and unlikely to cause trouble. Still, if you notice signs of a low (shaky, sweaty, dizzy) or higher-than-usual readings after adding guarana, mention it to your pharmacist or doctor before changing anything.

Theoretically, metformin might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Animal research shows that metformin can reduce caffeine metabolism. However, this effect has not been reported in humans.

Here's the gist: your Glucovance combo (glyburide plus metformin) isn't being changed by guarana. It's the other way around. Guarana is basically a concentrated caffeine source, and lab and animal studies hint that the metformin part may slow how your body clears caffeine. In theory that caffeine could linger and hit a little harder.

What that might feel like is the usual too-much-caffeine stuff: jitteriness, a racing or pounding heart, trouble sleeping, or feeling on edge. This has only been seen in animal research, not in people, so I wouldn't lose sleep over it (pun intended). Still, if you notice those symptoms or you have heart issues or anxiety, ease up on the guarana and check with your pharmacist or doctor first.
