Here's the simple version: Pemoline is a central nervous system stimulant, and guarana is basically a concentrated source of caffeine, which is also a stimulant. Stacking two stimulants together can add up, so you may feel more of the jittery side effects, like a racing or pounding heart, trouble sleeping, anxiety or restlessness, shakiness, and a higher blood pressure. Pemoline isn't a prodrug, so this isn't about one activating the other. It's simply two stimulants pushing in the same direction.

I'll be honest that this is a theoretical caution based on how these work, not strong proof from studies. A small amount of caffeine in food or drink is unlikely to be a big deal, but concentrated guarana supplements are another story. Please check with your pharmacist or doctor before combining them.
