Theoretically, guarana might increase the levels and clinical effects of pioglitazone.
Guarana contains caffeine. Animal research suggests that caffeine can modestly increase the maximum concentration, area under the curve, and half-life of pioglitazone, and also reduce its clearance. This increased the antidiabetic effects of pioglitazone. However, the exact mechanism of this interaction is unclear.

Your ActoPlus Met has two ingredients, and this interaction is really about the pioglitazone part. Guarana is basically a concentrated source of caffeine, and some early lab and animal research hints that caffeine may slow how your body clears pioglitazone. If that happens, pioglitazone could build up a bit and push your blood sugar lower than expected.

In plain terms, that could mean a higher chance of low blood sugar (shakiness, sweating, dizziness, feeling foggy). I want to be honest though: this is theoretical and only seen in animal studies, so it's far from certain in people. A cup of guarana tea probably matters less than concentrated energy supplements. Before adding guarana regularly, check in with your pharmacist or doctor.

Here's the gist: ActoPlus Met combines pioglitazone and metformin to help bring your blood sugar down. Guarana is basically a concentrated source of caffeine, and the worry on file is mostly theoretical. Caffeine can nudge blood sugar in either direction, so in theory a big guarana dose could make your numbers a little harder to predict while you're on these medicines.

The good news is this is rated minor and unlikely, and the evidence is on the weaker side. A cup of coffee or tea probably won't cause trouble. The bigger thing to watch is that guarana's caffeine can mask early low-blood-sugar warning signs like shakiness or a racing heart. If you use guarana regularly or in high doses, mention it to your pharmacist or doctor and keep an eye on your readings.

Here's the thing to know about pairing guarana with your ActoPlus Met (pioglitazone and metformin): the interaction on file actually runs in the opposite direction from what most people expect. It's not that your diabetes medicine becomes stronger or weaker. Instead, the metformin part may slow how your body clears the caffeine that guarana is loaded with, so the caffeine can hang around longer.

In plain terms, you might feel more of the usual caffeine effects: jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired. I want to be honest, though. This is based only on animal lab studies and has not actually been seen in people, so it's mostly theoretical and rated minor.
