Hemp and Acetaminophen, Aspirin, Caffeine

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Hemp interacts with whole categories of medication. Acetaminophen, Aspirin, Caffeine falls into 3 of those categories, so it can interact with Hemp in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 3

Why this applies to Acetaminophen, Aspirin, Caffeine

Acetaminophen, Aspirin, Caffeine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hemp interacts with.

Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for your Excedrin (which combines acetaminophen, aspirin, and caffeine). The aspirin piece is already a mild blood thinner, since it keeps platelets from clumping together. Hemp seed might nudge that same effect a little further, so in theory the combo could raise your bleeding risk slightly, think easier bruising, a nosebleed that takes longer to stop, or stomach upset.

I want to be honest about the strength of this, though. It rests on lab and animal data, and a human study using 2 grams of hemp seed oil daily for 12 weeks showed no real effect on platelets. So for most folks this is a minor, mostly theoretical concern. Still, if you use blood thinners or take Excedrin often, run it by your pharmacist first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Acetaminophen, Aspirin, Caffeine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that hemp induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS

Here's the piece that matters for Excedrin: the caffeine in it is broken down by a liver enzyme called CYP1A2. Lab studies (just test tube work, not people) hint that hemp might speed that enzyme up a bit. If that happened in real life, your body could clear caffeine a little faster, so you might notice slightly less of caffeine's usual kick, the part that helps boost the pain relief and perk you up.

Honestly, this is very theoretical and rated minor. The acetaminophen and aspirin parts aren't really affected this way. Food amounts of hemp seeds or hemp oil are unlikely to cause any real change. If you take concentrated hemp supplements regularly, it's worth a quick chat with your pharmacist, but there's no need to worry.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Acetaminophen, Aspirin, Caffeine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.

Let's keep this one in perspective. The concern on file is that hemp might speed up a liver enzyme called CYP3A4, which could in theory lower the levels of medicines that enzyme breaks down. With Excedrin, though, this is mostly a non-issue. Acetaminophen and aspirin aren't cleared in any meaningful way by CYP3A4, and the caffeine piece runs mainly through a different enzyme (CYP1A2). So even if hemp nudged CYP3A4, it wouldn't change how this combo works for your headache.

On top of that, this is only a theoretical idea from lab dish studies, not something seen in real people. Food amounts of hemp seeds or oil are very unlikely to matter. If you have questions, your pharmacist is happy to double-check with you.

About Hemp

What is Hemp?

Hemp
  • Plant-based protein source
  • Heart health support
  • Skin and hair health
  • Reducing inflammation
  • General nutrition (omega fatty acids)

Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.
