Hemp + Aspirin, Butalbital, Caffeine, Codeine Phosphate
Aspirin, Butalbital, Caffeine, Codeine Phosphate brand name: Fiorinal w/ Codeine.
Aspirin, Butalbital, Caffeine, Codeine Phosphate falls into 3 drug categories that Hemp can interact with; the most serious is rated minor. Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Hemp interacts with whole categories of medication. Aspirin, Butalbital, Caffeine, Codeine Phosphate falls into 3 of those categories, so it can interact with Hemp in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).
ANTICOAGULANT/ANTIPLATELET DRUGS
1 of 3Why this applies to Aspirin, Butalbital, Caffeine, Codeine Phosphate
Aspirin, Butalbital, Caffeine, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hemp interacts with.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the angle worth knowing: this combination product (Fiorinal with Codeine) already contains aspirin, which thins the blood a little by slowing down your platelets. Hemp has been looked at for a similar mild effect, so on paper, stacking the two could nudge your bleeding risk up. That might show as easier bruising, nosebleeds, or a cut that's slow to stop.
The honest part: this is mostly theoretical. The signal came from animal and lab studies, and a human trial using 2 grams of hemp seed oil daily for 12 weeks did not affect platelets. Food amounts of hemp seeds are very unlikely to matter. Still, if you use concentrated hemp supplements regularly, give your pharmacist a heads-up so we can keep an eye on things.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 3Aspirin, Butalbital, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
In vitro research shows that hemp induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece of this combo pill that matters most for the hemp question: caffeine. Of the four ingredients in Fiorinal with Codeine, caffeine is the one mainly cleared by the CYP1A2 enzyme. Lab studies (test tube only, not people) suggest hemp may speed up that enzyme a bit. In theory, that could lower caffeine levels and make its mild boost wear off a little faster. Honestly, this is a very minor and unproven concern.
The aspirin, butalbital, and codeine parts aren't really affected here. Everyday hemp seeds or hemp oil in food are very unlikely to cause any noticeable change. If you're using concentrated hemp supplements, it never hurts to run it by your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Aspirin, Butalbital, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that hemp induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Fiorinal with Codeine. Lab studies (not human studies) hint that hemp may speed up a liver enzyme called CYP3A4. The butalbital piece of this combo is partly handled by that enzyme, so in theory hemp could clear it a little faster and make it slightly less effective at easing your tension headache. Caffeine and aspirin aren't meaningfully touched here.
One nuance: codeine is a prodrug that the body turns into morphine mainly through a different enzyme (CYP2D6), so this CYP3A4 idea doesn't reliably change its pain relief. Honestly, this is very weak, theoretical evidence, and food amounts of hemp seeds are unlikely to matter. If you use concentrated hemp supplements, just mention it to your pharmacist.

From our pharmacist’s overview on the Aspirin, Butalbital, Caffeine, Codeine Phosphate page
“Fiorinal with Codeine packs four active drugs into one capsule, and I would treat its supplement list with genuine wariness rather than a quick skim. The aspirin component is why roughly half of the 398 ingredients on file raise bleeding risk, while the codeine and butalbital explain a second large cluster that deepens sedation, someti…”

About Hemp
What is Hemp?
- Plant-based protein source
- Heart health support
- Skin and hair health
- Reducing inflammation
- General nutrition (omega fatty acids)
Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.
