Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hemp interacts with.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.

Here's the idea behind this one. Both atazanavir and cobicistat rely heavily on a liver enzyme called CYP3A4. Cobicistat is actually in your combo on purpose, it blocks that enzyme so atazanavir stays at a high enough level to fight HIV. Some early lab research suggests hemp might do the opposite and rev up CYP3A4, which could in theory lower your atazanavir levels.

If that happened, the worry would be a weaker effect against the virus, which matters a lot for keeping HIV under control. That said, this is only test-tube evidence, not seen in real patients, and normal food amounts of hemp seeds or hemp oil are unlikely to cause trouble. Concentrated hemp supplements are more of a question mark. Please check with your pharmacist or doctor before adding one.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
