Hemp and Hydrocodone, Ibuprofen

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Hemp interacts with whole categories of medication. Hydrocodone, Ibuprofen falls into 2 of those categories, so it can interact with Hemp in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 2

Why this applies to Hydrocodone, Ibuprofen

Hydrocodone, Ibuprofen falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hemp interacts with.

Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing about Vicoprofen: it pairs hydrocodone (an opioid for pain) with ibuprofen, and that ibuprofen part already nudges your blood toward easier bleeding and can be a little hard on the stomach. Hemp comes into the picture because, at least in lab and animal studies, hemp seed may slightly slow down how platelets clump together. Stack that on top of ibuprofen and, in theory, you could see a touch more bruising, nosebleeds, or stomach irritation.

But let's keep this in perspective. This is rated a minor, mostly theoretical concern. In actual human research, hemp seed oil at 2 grams a day for 12 weeks did not affect platelets. Everyday food amounts of hemp seeds are very unlikely to cause trouble. If you take blood thinners or use concentrated hemp products, just check with your pharmacist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Hydrocodone, Ibuprofen also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS

Here's the gist with Vicoprofen (hydrocodone plus ibuprofen). Lab studies hint that hemp may speed up CYP3A4, one of the liver enzymes that helps process the hydrocodone part. If that happened in your body, your liver could clear the hydrocodone a little faster, which in theory might make the pain relief feel slightly weaker. The flip side: hydrocodone is partly turned into its more active form (hydromorphone) by a different enzyme, so the real-world effect here is hard to predict.

The good news is this is only a theoretical, lab-based concern, rated minor. Eating hemp seeds or hemp oil in food is very unlikely to matter. If you take concentrated hemp products regularly and notice your pain medicine working less well, just check in with your pharmacist or doctor before changing anything.

About Hemp

What is Hemp?

Hemp
  • Plant-based protein source
  • Heart health support
  • Skin and hair health
  • Reducing inflammation
  • General nutrition (omega fatty acids)

Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.
