Nintedanib Esylate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hemp interacts with.

Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.

Here's the thing to know about combining hemp with Ofev (nintedanib): nintedanib already carries its own bleeding risk on the label, separate from any supplement. The concern with hemp is that it might theoretically add a little to that risk by making platelets slightly less sticky. So in plain terms, stacking the two could, in theory, mean more bruising, nosebleeds, or bleeding that's slower to stop.

I want to be honest about how weak this concern is. It comes from animal and lab studies, and a human study using 2 grams of hemp seed oil daily for 12 weeks showed no effect on platelets. Food amounts of hemp seeds are very unlikely to matter. If you're taking concentrated hemp supplements alongside Ofev, just mention it to your pharmacist or doctor so they can keep an eye on you.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.

Here's the gist of this one: lab studies suggest hemp may speed up an enzyme called CYP3A4, which is one of the pathways your body uses to clear nintedanib (Ofev). If that happened, your nintedanib levels could drop a bit, which in theory might make it work less well at slowing your lung scarring. The good news is that CYP3A4 is only a minor route for this drug, so the effect is expected to be small.

Honestly, this is a theoretical concern based only on test-tube research, not real patients, so I wouldn't lose sleep over it. Eating hemp seeds as food is very unlikely to matter. If you're thinking about concentrated hemp supplements, just run it by your pharmacist or doctor first so they can keep an eye on how you're doing.

About Nintedanib

What Is Nintedanib Used For?

Brand name: Ofev

Nintedanib capsules are approved for adults with idiopathic pulmonary fibrosis (IPF) — a chronic, progressive scarring of the lungs — and for adults with chronic fibrosing interstitial lung diseases (ILDs) with a progressive phenotype, meaning fibrosing lung diseases that continue to worsen over time despite treatment.

Nintedanib capsules (including Ofev) are also indicated to slow the rate of decline in lung function in adults with systemic sclerosis-associated interstitial lung disease (SSc-ILD), a lung complication of the autoimmune condition systemic sclerosis (scleroderma). Note that the Nintedanib esylate capsule product is approved for IPF and chronic fibrosing ILDs with a progressive phenotype, but SSc-ILD approval is carried by Ofev and other nintedanib capsule products — always confirm with your prescriber which product you have been prescribed.
