Selumetinib Sulfate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hemp interacts with.

Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.

Here's the honest picture with hemp and Koselugo (selumetinib). The concern on file is about bleeding risk, since hemp seed could in theory make platelets a little less sticky. Selumetinib isn't a blood thinner in the usual sense, but it can lower platelet counts and cause bleeding or easy bruising on its own, so anything that adds to that is worth a glance.

The good news: the evidence here is very weak. It comes from lab and animal studies, and a human study using hemp seed oil 2 grams a day for 12 weeks did not thin the blood. Food amounts of hemp seeds are very unlikely to matter. Still, since selumetinib already affects bleeding, mention any hemp supplements to your child's care team before starting them.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.

Here's the situation with hemp and Koselugo (selumetinib). Selumetinib is cleared from the body partly by a liver enzyme called CYP3A4. Some early lab research hints that hemp might speed that enzyme up, which could in theory clear selumetinib out a little faster and make it slightly less effective at slowing those NF1 tumors. Selumetinib is the active drug itself, not a prodrug, so this would mean a bit less benefit, not more side effects.

I want to be honest though: this is only test-tube data, the concern is rated minor, and we don't have real human cases showing a problem. Eating hemp seeds or oil in food is very unlikely to matter. Before using concentrated hemp supplements, just check with your child's oncology team or pharmacist.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
