Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the gist for clomipramine: your liver uses an enzyme called CYP2D6 to help clear this medication out of your body. Rauvolfia vomitoria contains a small amount of yohimbine, which in lab studies appears to slow that same enzyme down. If that happens in real life, clomipramine could build up a bit higher than usual, which may mean stronger side effects like drowsiness, dry mouth, constipation, dizziness, a faster heartbeat, or feeling more sedated.

I want to be honest about the evidence though: this is theoretical, based only on test-tube research, not on people. So it may not amount to much. Still, this plant has its own powerful effects on blood pressure and mood, so please loop in your doctor or pharmacist before combining the two.

Clomipramine also falls under TRICYCLIC ANTIDEPRESSANTS (TCAs).

Theoretically, concomitant use might alter the effects of Rauvolfia vomitoria and increase the risk for adverse effects.
A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine, a constituent of Rauvolfia vomitoria, 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor. Also, concomitant use of TCAs with Rauvolfia vomitoria may decrease the effects of rauwolfia alkaloids.

Here's the practical takeaway if you're on clomipramine (Anafranil) and thinking about Rauvolfia vomitoria. This plant carries a mix of strong compounds, including yohimbine-like ones. In a small study, people already taking a tricyclic antidepressant who were given yohimbine ran into severe anxiety, agitation, and tremor. Since clomipramine itself can already cause jitteriness, restlessness, a racing heart, and shakiness, adding this herb could pile on those same effects.

On the flip side, the plant's blood-pressure-lowering alkaloids (like reserpine) may work less well when a TCA is on board. This is mostly theoretical, based on how these substances behave rather than solid trials, so don't panic. Just please run it past your pharmacist or doctor before combining them.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
