Why this applies to Crizotinib

Crizotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist: your body uses a liver enzyme called CYP3A4 to break down crizotinib. In a test tube, red clover looked like it might slow that enzyme down, which in theory could let crizotinib build up a bit. Since crizotinib already comes with side effects like nausea, vision changes, swelling, liver enzyme bumps, and slowed heart rate, higher levels are something we'd rather avoid.

The reassuring part: when red clover was actually tested in people using a similar CYP3A4 drug, it didn't change drug levels. So this concern stays mostly theoretical and unlikely. Still, because crizotinib is a serious cancer medicine, please run any red clover use past your oncology team or pharmacist first.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. What Is Crizotinib Used For?

Brand name: Xalkori

Xalkori (crizotinib) treats adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for ALK or ROS1 gene changes, as confirmed by an FDA-approved test.

Xalkori is also used for pediatric patients (age 1 and older) and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL), and for adult and pediatric patients (age 1 and older) with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
