ADO-Trastuzumab Emtansine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yohimbe interacts with.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with Kadcyla (ado-trastuzumab emtansine) and yohimbe. The concern on file is that yohimbe might speed up a liver enzyme called CYP3A4. The part of Kadcyla that actually kills cancer cells, a piece called DM1, is handled in part by that same enzyme. So in theory, revving it up could lower how much active drug sticks around and slightly weaken the treatment.
That said, this is very weak evidence, just lab and test-tube data, not real people, and it's rated minor. I wouldn't lose sleep over it. But Kadcyla is serious cancer therapy, so don't add yohimbe (which can also raise blood pressure and heart rate) without first checking with your oncology team or pharmacist.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

What Is Ado-trastuzumab Emtansine Injection Used For?
Brand name: Kadcyla
Kadcyla is used by itself (as a single agent) for HER2-positive breast cancer in two situations. Your tumor must test positive for HER2 using an FDA-authorized test.
- Metastatic breast cancer: for people who already received trastuzumab and a taxane, and who either had treatment for metastatic disease or whose cancer returned during or within six months of finishing adjuvant (after-surgery) therapy.
- Early breast cancer: as adjuvant treatment for people who still have residual invasive disease after neoadjuvant (pre-surgery) taxane and trastuzumab-based treatment.
