Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the gist for Rexulti (brexpiprazole). Your liver uses an enzyme called CYP2D6 to help clear this medicine out of your system. Yohimbe contains yohimbine, and in lab studies it appears to slow that same enzyme down. If that happens in the body, brexpiprazole could build up a bit higher than expected, which may mean stronger effects and more side effects like drowsiness, dizziness, restlessness (feeling unable to sit still), or weight gain.

The honest part: this concern comes only from test-tube research, not from real patients, so it's more of a theory than a proven problem. Still, yohimbe on its own can raise blood pressure and heart rate, so please check with your pharmacist or doctor before combining the two.

Here's the gist for Rexulti (brexpiprazole). Your liver uses an enzyme called CYP3A4 to help clear this medication. Lab studies suggest yohimbe may speed that enzyme up a bit, which in theory could push brexpiprazole out of your system faster and lower its levels. If that happened, the drug might not work as well, so you could notice your symptoms (like mood, thinking, or depression) creeping back.

The good news: this is only a theoretical concern based on test-tube data, not on real patients, and it's rated minor. Brexpiprazole isn't a prodrug, so it needs steady levels to do its job. Yohimbe also carries its own risks for blood pressure and heart rate, so please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
