Here's the gist with Auvelity (the bupropion and dextromethorphan combo). The dextromethorphan part is cleared from your body partly by a liver enzyme called CYP2D6. Lab studies hint that yohimbine, the active piece of yohimbe, may slow that enzyme down. If that happens, dextromethorphan could build up a bit, which might mean more dizziness, drowsiness, nausea, or feeling foggy. Bupropion itself can also nudge blood pressure and heart rate up, and yohimbe is well known for doing the same, so stacking them could leave you feeling jittery, anxious, or racing.

Keep in mind this enzyme concern is theoretical and only comes from test-tube research, not real people. Still, given yohimbe's safety reputation, please run it past your pharmacist or doctor before combining them.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the key thing with Auvelity (the bupropion plus dextromethorphan combo): bupropion is actually one of the stronger blockers of the liver enzyme called CYP2D6. Yohimbe's active ingredient, yohimbine, is cleared from your body by that very same enzyme. So when you slow that enzyme down with bupropion, yohimbine can't be broken down as quickly and it tends to build up in your bloodstream.

What that means in practice is more of yohimbe's stimulant-type effects, things like a racing or pounding heart, jitteriness, anxiety, and a spike in blood pressure. That's worth taking seriously since this combo is fairly likely to happen. Please don't change your Auvelity dose, but do check with your pharmacist or doctor before adding yohimbe, especially since over-the-counter products vary a lot in strength.

Auvelity combines dextromethorphan and bupropion, and it's prescribed for depression. Here's the gist of this one: lab studies (just test-tube work, not people) suggest yohimbe might speed up a liver enzyme called CYP3A4. That enzyme is involved in clearing dextromethorphan, so in theory yohimbe could lower its levels and make this medicine work a little less well.

Honestly, this is a minor, theoretical concern based on weak evidence, so I wouldn't lose sleep over it. The bigger worry with yohimbe is its own side effects, like a racing heart and spikes in blood pressure, which aren't great to stack on top of bupropion. Please run it by your pharmacist or doctor before adding yohimbe.
