Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the thing to know about pairing yohimbe with darunavir. Darunavir is always taken with a booster like ritonavir or cobicistat, and those boosters are strong blockers of an enzyme called CYP3A4. That same enzyme is what your body uses to break down yohimbine, the active part of yohimbe bark. So when you slow that enzyme down, yohimbine can pile up in your system more than it normally would.

In plain terms, this could make yohimbe's own side effects hit harder, things like a racing heartbeat, jitteriness, anxiety, and spikes in blood pressure. This concern is mostly theoretical, based on how the drugs are processed rather than reports of harm, but yohimbe products are already unpredictable. CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the thing to understand: in this pairing it's the yohimbe that gets affected, not your HIV medicine. Darunavir (and the booster it's taken with, like ritonavir or cobicistat) blocks a liver enzyme called CYP2D6, which happens to be one of the main enzymes that clears yohimbine out of your body. When that pathway is slowed down, yohimbine can pile up to higher levels than usual.

What that means for you is a bigger chance of yohimbe's rougher side effects, things like a racing heartbeat, spikes in blood pressure, anxiety, jitteriness, and trouble sleeping. Your darunavir keeps working normally for HIV. Since over-the-counter yohimbe is already unpredictable, please check with your pharmacist or doctor before combining them rather than guessing.

Here's the basic idea with darunavir (Prezista) and yohimbe. Darunavir is cleared by a liver enzyme called CYP3A4, which is also why it's always paired with a booster like ritonavir or cobicistat to keep its levels high enough to work. Lab studies suggest yohimbe might speed up that same enzyme, which in theory could lower your darunavir levels. If that happened, your HIV treatment could become less effective, and over time that risks the virus rebounding or becoming resistant.

I want to be honest though: this is only a theoretical concern from test-tube research, not real patients, and it's rated minor. Still, because keeping HIV controlled matters so much, I'd run any yohimbe product by your pharmacist or doctor before starting it.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Darunavir

What Is Darunavir Used For?

Brand name: Prezista

Darunavir treats HIV-1 infection in adults and in children who are at least 3 years old and weigh at least 10 kg (about 22 lbs). It is not a cure for HIV, but it is used as part of a combination antiretroviral regimen to control the virus.

Darunavir must always be taken together with ritonavir — that pairing is required for darunavir to work properly. Both Prezista (brand name) and generic darunavir tablets are approved for this use. Prezista is also available as an oral suspension, which is particularly useful for younger or smaller patients.
