Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Yohimbe interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the thing to know about pairing yohimbe with fosaprepitant (Emend Injection). Fosaprepitant is the IV form used to prevent nausea and vomiting from chemotherapy, and once it's in your body it turns into aprepitant, which gently blocks a liver enzyme called CYP3A4. Yohimbine, the active part of yohimbe, is broken down by that same enzyme. So in theory, fosaprepitant could slow the clearance of yohimbine and let it build up in your system.

If that happens, you might feel yohimbe's effects more strongly: a racing heart, jumpiness, anxiety, or a spike in blood pressure. I'll be honest, this concern is theoretical and based on how the drugs work, not real-world reports. Still, please check with your pharmacist or doctor before combining them.

Here's the gist for fosaprepitant (Emend Injection), the IV medicine doctors give to help prevent the nausea and vomiting that come with chemotherapy. Fosaprepitant is converted in your body into aprepitant, which is handled by a liver enzyme called CYP3A4. Lab studies suggest yohimbe might speed that enzyme up. In theory, that could clear the drug a little faster and make it slightly less effective at keeping nausea away.

I want to be honest about how soft this evidence is. It comes only from test-tube research, the interaction is rated minor, and fosaprepitant is usually a one-time hospital dose. Still, since yohimbe itself can raise blood pressure and heart rate, it's worth telling your oncology team before using it.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.
