Ondansetron falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the quick picture: your body uses an enzyme called CYP2D6 to help clear ondansetron. Lab studies suggest yohimbine (the active part of yohimbe) can slow that enzyme down a bit. In theory, that could let a little more ondansetron build up than usual, which might nudge up its typical side effects like headache, constipation, or feeling tired. One thing worth knowing is that ondansetron can affect heart rhythm (a change called QT prolongation), and yohimbe itself can speed up the heart and raise blood pressure, so stacking the two isn't ideal.

That said, this concern comes only from test-tube research, not real people, so it's more theoretical than proven. Still, given yohimbe's own heart risks, I'd run it past your pharmacist or doctor before combining them.

Here's the basic idea: ondansetron (Zofran) is partly cleared by a liver enzyme called CYP1A2. Lab studies suggest yohimbe may speed up that enzyme. If it does, your body could break down ondansetron a little faster, which in theory might make it slightly less effective at controlling nausea and vomiting. Ondansetron is the active drug itself, not a prodrug, so faster clearance means weaker effect, not stronger.

I want to be honest about the strength here: this is only test-tube research, the interaction is rated minor, and we have no real human evidence it actually matters. So I wouldn't lose sleep over it. Still, since over-the-counter yohimbe carries its own risks like raising blood pressure and heart rate, please run it by your pharmacist or doctor first.

Here's the gist for ondansetron (Zofran): your liver uses an enzyme called CYP3A4 to help clear it out. Lab studies suggest yohimbe may speed up that enzyme, which could break ondansetron down a little faster than usual. In theory, that might leave slightly less of the drug working, so its anti-nausea effect could be a touch weaker.

I want to be honest about how soft this is. Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Ondansetron

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
