Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Regorafenib
Regorafenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yohimbe interacts with.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version for Stivarga (regorafenib). Your body relies on a liver enzyme called CYP3A4 to help process this cancer medicine. Lab studies (done in test tubes, not in people) hint that yohimbe may speed that enzyme up. If that happened in real life, it could clear regorafenib out a bit faster and potentially make it work less well against your cancer, which is the last thing you want with a treatment this important.
That said, this is theoretical and based only on early lab data, so we don't know if it truly matters in a person. Given how serious the reason for taking regorafenib is, and yohimbe's own risks like high blood pressure and a racing heart, please check with your oncologist or pharmacist before combining them.

From our pharmacist’s overview on the Regorafenib page
“Regorafenib (Stivarga) is a targeted oral cancer therapy, and with a drug like this the goal is keeping its blood level inside a fairly tight window, so the 138 supplements and herbs on this page deserve real respect. Nearly all of them, 121 in total, can shift how the liver enzyme CYP3A4 processes the drug, nudging its level up or down.”

About Yohimbe
What is Yohimbe?
- Erectile dysfunction
- Low sex drive
- Sexual performance
- Weight loss
- Athletic performance
Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.
About Regorafenib
What Is Regorafenib Used For?
Brand name: Stivarga
Stivarga is approved to treat three types of advanced cancer in adults. It is used for metastatic colorectal cancer (cancer that has spread) after prior chemotherapy and targeted therapy; for locally advanced, unresectable, or metastatic gastrointestinal stromal tumor (GIST) after two previous treatments; and for hepatocellular carcinoma (liver cancer) after treatment with sorafenib.
All three uses are specific to Stivarga tablets taken by mouth. It is intended for patients whose cancer has progressed despite earlier lines of therapy.
