Rolapitant Hydrochloride falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS — the group Yohimbe interacts with.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the thing to know about pairing these two: your body relies on an enzyme called CYP2D6 to break down yohimbine, the active part of yohimbe. Rolapitant (Varubi) happens to block that same enzyme. So when they're used together, the yohimbine can't clear out as quickly and may pile up to higher levels than expected.

In plain terms, this could amplify yohimbe's own side effects, things like a racing heartbeat, jittery or anxious feelings, and elevated blood pressure. Notice that it's the yohimbe that gets stronger here, not your anti-nausea treatment. Since over-the-counter yohimbe is already unpredictable and rolapitant's enzyme-blocking effect can linger for weeks, I'd really suggest checking with your pharmacist or doctor before combining them.

Here's the gist for Varubi (rolapitant), the medicine that helps hold off the delayed nausea and vomiting that can hit days after chemo. Rolapitant is handled in part by a liver enzyme called CYP3A4. Some lab research suggests yohimbe may speed that enzyme up. If that happened in real life, your body could clear rolapitant a little faster, which in theory might make it work less well, meaning more breakthrough nausea or vomiting when you're counting on it most.

I want to be honest about the strength here: this is a minor, theoretical concern based only on test-tube data, not real patient cases. Still, with chemo support I don't like to gamble. Yohimbe also carries its own risks, like raising blood pressure and heart rate, so please run it by your oncology team or pharmacist before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
