Triclabendazole falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS — the group Yohimbe interacts with.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the thing to keep in mind if you're taking triclabendazole (Egaten) for a liver fluke infection and thinking about a yohimbe supplement. Triclabendazole can act as a CYP2D6 inhibitor, and CYP2D6 is the same enzyme your body uses to clear yohimbine, the active piece of yohimbe. So triclabendazole isn't the drug that gets affected here, it's the yohimbe that can pile up. With slower breakdown, yohimbine levels rise and so do its side effects.

Practically, that can mean a racing or pounding heart, spikes in blood pressure, anxiety, sweating, or trouble sleeping. Triclabendazole is usually a short course, so the overlap may be brief, but yohimbe is unpredictable on its own. Please run this by your pharmacist or doctor before combining them.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP1A2 substrates.

Here's the situation with these two. Lab studies hint that yohimbe may speed up an enzyme in your liver called CYP1A2, which is one of the pathways that helps process triclabendazole (Egaten). If that enzyme runs faster, your body could clear the medicine a bit quicker, leaving slightly less of it around to do its job killing the liver flukes.

I want to be honest about how soft this is. It comes only from test-tube research, it's rated a minor concern, and triclabendazole is usually taken as just one or two doses with food. So a real-world problem is unlikely. Still, since yohimbe itself can raise blood pressure and heart rate, please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
