Voriconazole falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Yohimbe interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the gist for this pair: voriconazole is a strong blocker of a liver enzyme called CYP3A4, and that same enzyme is what your body uses to break down yohimbine, the active part of yohimbe. So if you take them together, your body may clear the yohimbine more slowly, letting it build up to higher levels than expected.

Practically, that means the unpleasant effects of yohimbe could hit harder, things like a racing heart, spikes in blood pressure, anxiety, jitteriness, and trouble sleeping. I'll be honest, this is a theoretical concern based on how these compounds are processed, not something proven in big studies, so don't panic. Still, yohimbe is unpredictable on its own. Please check with your pharmacist or doctor before combining them.

Here's the theory behind pairing yohimbe with voriconazole. Lab studies suggest yohimbe may speed up an enzyme called CYP3A4, and voriconazole is partly broken down by that same enzyme. If yohimbe revs up that pathway, your body could clear voriconazole a bit faster, which might lower its levels. Since voriconazole is an antifungal that has to stay at the right level to keep a serious infection in check, weaker drug levels could mean it works less well.

That said, this is only a theoretical concern from test-tube research, not from people, so it may not matter much in real life. Still, because voriconazole treats serious infections, it's worth running any yohimbe product past your pharmacist or doctor first.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats serious fungal infections. These include invasive aspergillosis (a serious mold infection), candidemia (Candida yeast in the bloodstream) in people who are not neutropenic, and other deep tissue Candida infections.

It is also used for esophageal candidiasis (a Candida infection of the esophagus) and for serious infections caused by Scedosporium apiospermum and Fusarium species in people who cannot tolerate other treatment or whose infection has not responded to it. Esophageal candidiasis is listed for most products, but not for the Voriconazole for injection (intravenous) product in the supplied evidence. Age limits differ by product.
