Why this applies to Simvastatin

Simvastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with simvastatin: your body relies on an enzyme called CYP3A4 to break it down. Danshen seems to have a split personality with this enzyme. In a small study of healthy men, taking it daily for 10 to 14 days sped up that enzyme in the gut, which would clear simvastatin faster and could make it work less well at lowering your cholesterol. But a single dose did the opposite, slowing clearance and raising drug levels, which could push up the risk of muscle aches or, rarely, more serious muscle problems.

This is early evidence, not a hard rule, so don't panic. Still, since the effect can swing both ways, please check with your pharmacist or doctor before pairing Danshen with simvastatin.

Simvastatin also falls under GLUCURONIDATED DRUGS.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the honest picture with Danshen and simvastatin. The concern on file involves glucuronidation, one of the ways your body packages up certain drugs for removal. In lab and animal studies, Danshen sends mixed signals: it can speed up these enzymes in some settings and slow them down in others, partly depending on how the root was processed. The bottom line is we can't say for sure which way it would push simvastatin levels, and this has never actually been reported in people.

If Danshen did raise simvastatin levels, the worry would be muscle aches, weakness, or rarely liver irritation. Because this is theoretical and your blood thinning is also a factor with Danshen, please loop in your pharmacist or doctor before combining them. Don't stop your statin on your own.

About Simvastatin

What Is Simvastatin Used For?

Brand name: Zocor

Simvastatin tablets are used alongside a cholesterol-lowering diet to treat high LDL cholesterol (bad cholesterol), high triglycerides, and certain inherited cholesterol disorders, including heterozygous familial hypercholesterolemia (HeFH) in adults and children aged 10 and older, and homozygous familial hypercholesterolemia (HoFH) in adults.

In adults who already have coronary heart disease, cerebrovascular disease, peripheral vascular disease, or diabetes and are at high risk for heart events, simvastatin is also used to reduce the risk of death from heart disease, non-fatal heart attack, stroke, and the need for procedures to open blocked arteries. Zocor is one brand-name product containing simvastatin and carries these same indications.
