Acetaminophen, Codeine, Doxylamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

First, the big picture: ephedra itself is risky and banned in U.S. supplements because of its strong stimulant effects on the heart and blood pressure. So I wouldn't pair it with anything. The specific enzyme issue here is a minor footnote by comparison.

The interaction on file says ephedra might speed up an enzyme (CYP1A2) that helps clear certain drugs. But this is only based on animal and lab work, so it's theoretical and weak. CYP1A2 isn't the main pathway for the codeine, acetaminophen, or doxylamine in Mersyndol, so I wouldn't expect a meaningful change in pain relief, drowsiness, or other effects from that enzyme alone. The real concern is ephedra's stimulant load fighting the sedating ingredients. Please run this by your pharmacist before combining them.

Here's the concern with combining ephedra and a product like Mersyndol (which contains acetaminophen, codeine, and doxylamine). The worry centers on the acetaminophen part. Acetaminophen is processed by your liver, and in high doses or with other liver stressors it can cause liver damage. Ephedra supplements have been tied to many real cases of liver injury on their own. So the idea is that pairing them might add up and put extra strain on your liver.

To be honest, this is a theoretical concern based on how each acts, not proven cause-and-effect for this combo. Still, ephedra is banned in U.S. supplements for good reason, and I wouldn't recommend it. Please check with your pharmacist or doctor before using either, and watch for yellowing skin, dark urine, or belly pain.
