Theoretically, guarana might increase the levels and clinical effects of pioglitazone.
Guarana contains caffeine. Animal research suggests that caffeine can modestly increase the maximum concentration, area under the curve, and half-life of pioglitazone, and also reduce its clearance. This increased the antidiabetic effects of pioglitazone. However, the exact mechanism of this interaction is unclear.

Your medication Oseni combines two diabetes drugs, and the part we're focused on here is the pioglitazone. The caffeine in guarana may, in theory, slow down how fast your body clears pioglitazone, letting a bit more of it linger in your system. The practical worry is that this could nudge its blood-sugar-lowering effect a little stronger than expected, which raises the chance of low blood sugar (shakiness, sweating, dizziness, confusion).

Now, the honest part: this is based only on animal research, so it's more of a theoretical heads-up than a proven problem in people. An occasional cup of coffee or tea isn't likely to cause trouble. Still, before using concentrated guarana or energy supplements regularly, run it by your pharmacist or doctor.

Both ingredients work to lower your blood sugar, and guarana's main active part is caffeine. The concern on file is purely theoretical and based on mixed reports: caffeine might nudge your blood sugar up or down a little, which could make your numbers harder to read while you're on this medicine.

The good news is this is rated a minor, unlikely interaction. Guarana doesn't block or speed up how Oseni is cleared, so it won't make the drug itself stronger or weaker.
