Cryptenamine, Methyclothiazide falls under DIURETIC DRUGS — the group Guarana interacts with.

Theoretically, using guarana with diuretic drugs might increase the risk of hypokalemia.
Guarana contains caffeine. Caffeine, especially in excessive amounts, can reduce potassium levels due to stimulation of the sodium-potassium pump. Diuretics can also lower potassium levels.

Your Diutensen contains methyclothiazide, a thiazide-type water pill. One thing thiazides are known for is nudging your potassium level down. Here's the concern with guarana: it's basically a concentrated source of caffeine, and large amounts of caffeine can also push potassium lower by stimulating the pump that moves potassium into your cells. Stack the two together and you could, in theory, end up with low potassium (hypokalemia), which can show up as muscle cramps, weakness, or an irregular heartbeat.

I'll be honest with you, this concern is based on how the two work on paper, not on strong studies, so the risk is modest. An occasional cup of coffee or tea is unlikely to matter. Daily high-dose energy or guarana supplements are the bigger question. Check with your pharmacist or doctor, and let them know if you feel unusually weak or shaky.
