Here's the honest picture: this isn't really about how your body handles linagliptin (Tradjenta) itself. Guarana won't speed up or slow down the way your liver clears this medicine. The concern is simpler. Guarana is basically a concentrated caffeine source, and caffeine can nudge your blood sugar around, sometimes up and sometimes down, depending on the person.

So the worry is that guarana could make your blood sugar a little less predictable while you're working to control it with linagliptin. The evidence here is weak and conflicting, and this is rated a minor, unlikely interaction. An occasional energy drink or soda probably won't throw things off. If you start using guarana supplements regularly, just give it a mention to your pharmacist or doctor and keep an eye on your blood sugar readings.
