Lomefloxacin falls under QUINOLONE ANTIBIOTICS — the group Guarana interacts with.

Theoretically, quinolone antibiotics might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Theoretically, concomitant use might increase serum caffeine concentrations and the risk of adverse effects. Quinolones (also called fluoroquinolones) can decrease caffeine clearance by inhibiting the cytochrome P450 1A2 enzyme.

Here's the thing to know if you're pairing guarana with lomefloxacin: it's really the caffeine in guarana that matters. Lomefloxacin belongs to the fluoroquinolone family, and these antibiotics can slow down a liver enzyme (CYP1A2) that normally clears caffeine from your body. So the caffeine in guarana can hang around longer than usual.

In plain terms, your normal guarana dose may hit harder. You might feel jittery, have a racing or pounding heart, trouble sleeping, or feel more anxious. The antibiotic itself isn't being changed, so this isn't dangerous for most people, and the evidence here is fairly modest. Still, if you notice you're wired or restless while on lomefloxacin, easing back on guarana and other caffeine sources is a smart move. Check with your pharmacist if you're unsure.
